Job Summary

A company is looking for a Social Worker - LCSW.

Key Responsibilities

• Provide integrated medical, behavioral, and social care to patients with complex health care needs
• Deliver brief treatment for common mental health issues and consult with group residences on behavioral health functioning
• Document patient encounters and facilitate treatment plan changes based on patient progress and needs

Required Qualifications

• Master's degree in Social Work
• Current New York LCSW required
• 2+ years of prior healthcare/behavioral health experience required
• Geriatric care experience highly desired
• Proficient in computer skills and electronic health record documentation
