Key Responsibilities

• Provide integrated medical, behavioral, and social care to patients with complex health needs
• Deliver brief evidence-based treatment for mental health issues and support psychotropic medication management
• Document patient assessments and collaborate with the clinical team to adjust treatment plans as necessary

Required Qualifications, Training, and Education

• Master's degree in Social Work
• Current New York LCSW required
• 2+ years of prior healthcare/behavioral health experience required
• Geriatric care experience highly desired
• Proficient in computer skills and electronic medical record documentation
