New York Licensed Social Worker

Job Summary

A company is looking for a Social Worker - Remote.

Key Responsibilities:

• Validate referrals and collaborate with teams to assess member needs and identify gaps in care
• Build therapeutic relationships and guide conversations to align with assessment objectives
• Coordinate and deliver services, adapting strategies to meet member needs and document care plans

Required Qualifications:

• Bachelor's degree in Social Work
• 2+ years of Behavioral Health / Substance Use Disorder experience
• 2+ years of experience in the health care industry, including clinical experience
• Intermediate proficiency using a PC in a Windows environment, including Microsoft Word and Excel
