Job Summary

A company is looking for a Social Worker Care Manager to service patients in the Long Island, NY area.

Key Responsibilities

• Support the mental and physical health care of patients on an assigned caseload
• Screen and assess patients for mental health and substance abuse disorders, facilitating engagement and follow-up care
• Document patient progress and treatment recommendations in EHR and other required systems

Required Qualifications

• Minimum of a Master's in Social Work (MSW)
• Preferred Licensure or provisionally licensed
• Behavioral health experience and knowledge
• Must have a valid driver's license and an insured automobile in good working order
• Time management skills are essential
