Job Summary

A company is looking for a Social Worker (Remote) Per Diem.

Key Responsibilities

• Provides culturally competent direct service to patients and families using various social work modalities
• Utilizes high risk criteria to identify patients needing social work assessment and advocates for their healthcare access
• Formulates and implements treatment and discharge plans, incorporating community resources and maintaining documentation

Required Qualifications, Training, and Education

• Master of Social Work required
• Current License to practice as a Master Social Worker or Clinical Social Worker in New York State required
• 1-3 years of relevant experience required
• Specialized certifications as needed
