Job Summary
Delivering comprehensive virtual care, the full-time New York Licensed Telehealth Provider (Nurse Practitioner/Physician Assistant) will engage in patient assessments, condition management, and medication review while working remotely.
Key responsibilities
- Provide clinical care delivery and care coordination, ensuring compliance with telehealth regulations
- Educate patients on preventive care and healthy lifestyle behaviors to enhance health outcomes
- Participate in quality improvement initiatives while adhering to clinical guidelines and regulatory standards
Required qualifications
- Active, unrestricted New York State clinical license as a Nurse Practitioner or Physician Assistant
- Minimum of 3600 hours of practice experience or 3 years of clinical experience in a primary care setting
- Board Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant
- Active Drug Enforcement Administration (DEA) license
- Current Basic Life Support Certification for health care providers
