Job Summary
Providing telephonic and telehealth care, the full-time New York Licensed Telephonic Nurse Practitioner will manage patient care coordination, conduct assessments, and collaborate with healthcare teams while working remotely with a flexible schedule that includes evenings, nights, and rotating weekends.
Key responsibilities
- Deliver telephonic care and coordinate with caregivers, facilities, and providers to manage patient needs
- Perform comprehensive assessments and document findings in compliance with healthcare regulations
- Facilitate care transitions and monitor patient plans to ensure quality and cost-effective care delivery
Required qualifications
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in New York and the state of residence
- Board certification as a Nurse Practitioner through a national board
- Current DEA licensure or ability to obtain post-hire as per state regulations
- Proficient in electronic medical records and documentation practices
