New York Oncology Nurse Navigator

Job Summary

A company is looking for an Oncology Nurse Navigator.

Key Responsibilities

• Provide triage, support, and education to members throughout their cancer journey via various communication methods
• Conduct comprehensive clinical assessments and facilitate end-of-life care discussions while managing member health
• Coordinate care and address member needs, including symptom management and provider referrals, while collaborating with healthcare teams

Required Qualifications

• Bachelor of Science Degree in Nursing and an unrestricted registered nurse (RN) license
• At least 5 years of nursing experience, with 3 years in solid tumor oncology nursing
• Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM required or to be obtained within 2 years of hire
• Compact and/or single state licensure in New York and/or Illinois preferred
