Job Summary
To support a compassionate cancer care experience, the full-time Oncology Nurse Navigator will provide triage, support, and education to members throughout their cancer journey, working remotely and engaging in clinical assessments and care coordination.
Key responsibilities
- Conduct comprehensive clinical assessments and manage member health, addressing clinical issues and facilitating end-of-life care discussions
- Establish trusting relationships with members and their care network, ensuring adherence to care protocols and documentation standards
- Collaborate with healthcare teams to monitor member progress and provide targeted support throughout the oncology care continuum
Required qualifications
- Active, unrestricted compact Registered Nurse (RN) license; degree in Nursing (ADN or BSN) required, BSN preferred
- At least 5 years of nursing experience, with a minimum of 3 years in solid tumor oncology nursing
- Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM required or to be obtained within 2 years of hire
- Compact and single-state licensure in New York and/or Illinois preferred
- Fluency in Spanish, both written and verbal, is preferred
