Job Summary
New York Patient Navigator, a full-time position, is responsible for supporting immigrant survivors of torture by helping them navigate healthcare and community systems, providing hands-on assistance, and facilitating access to resources.
Key Responsibilities
- Serve as the assigned Client Navigator for a panel of clients, maintaining regular contact and ensuring understanding of care plans
- Co-facilitate orientation and health education groups while promoting community engagement and resource access
- Participate in case discussions and team meetings, documenting encounters and communicating with clinical staff for coordinated support
Required Qualifications
- Lived experience as a refugee, asylee, or migrant
- Fluency in English and at least one additional language spoken by the clinic's patient population
- Commitment to supporting others and fostering trust in healthcare and community systems
- Experience working or volunteering in a community, social service, or health setting (preferred)
- Familiarity with NYC community resources and healthcare navigation (preferred)
