New York RN Care Manager

Job Summary

A company is looking for an RN - Care Manager (Bilingual).

Key Responsibilities

• Manage care planning, coordination, and health assessments for patients
• Monitor the quality and effectiveness of services and track progress towards individual goals
• Oversee patient transitions and facilitate discharge planning from hospitals or nursing homes

Required Qualifications, Training, and Education

• Valid NYS RN License
• Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings
• Associate's degree or higher is required
• Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole
• Ability to effectively navigate and utilize various software and tools relevant to care management
