New York RN Care Manager

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Evaluate member needs and recommend care plans for optimal outcomes
• Develop ongoing care plans and collaborate with providers to address unmet needs
• Provide education to members and families on healthcare options and resources

Required Qualifications, Training, and Education

• Active New York State RN license required
• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience preferred
• Knowledge of care management interventions and healthcare resources
• Ability to document and maintain compliance with regulations
