Job Summary

A company is looking for an RN Care Manager - Hybrid/Remote.

Key Responsibilities

• Manage care for patients with chronic diseases, focusing on disease education and health outcomes
• Coordinate services and collaborate with provider offices to ensure integrated care
• Identify and mitigate social influencers of health affecting patient care

Required Qualifications

• Associates Degree required; BSN preferred
• Minimum of 5 years of RN experience
• Previous experience in care management in relevant settings
• Certification in Case Management (CCM) preferred
• Prior electronic medical record (EMR) experience required
