New York RN Care Manager

Job Summary

Offering a fully remote opportunity, the full-time New York RN Care Manager will manage care planning, coordination, and health assessments for a diverse patient population while residing within the New York Tri-State Area.

Key responsibilities

• Manage care planning and coordination, ensuring quality and effectiveness of services
• Oversee patient transitions and facilitate smooth discharge planning from hospitals or nursing homes
• Identify health, environmental, and psychosocial risks, recommending interventions in collaboration with the Interdisciplinary Care team

Required qualifications

• Valid NYS RN License
• Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings
• Associate's degree or higher
• Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole
• Ability to effectively navigate and utilize various software and tools relevant to care management
