New York RN Care Manager

Job Summary

To support a diverse patient population, the full-time RN Care Manager will manage care planning, coordination, and health assessments while working in a hybrid environment and residing within the New York Tri-State Area.

Key responsibilities

• Manage care planning and coordination, ensuring the highest level of care for patients
• Monitor service quality and track patient progress towards individual health goals
• Facilitate smooth transitions and discharge planning for patients from hospitals or nursing homes

Required qualifications

• Valid NYS RN License
• Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings
• Associate's degree or higher
• Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole
• Ability to navigate and utilize various software and tools relevant to care management
