New York RN Care Navigator

Job Summary

A company is looking for an RN Care Navigator (100% Remote for New York residents).

Key Responsibilities

• Manage case management specific to kidney health, including member data entry and care plan maintenance
• Educate members on kidney health, dialysis, and transplant options while engaging them in the Kidney Health Program
• Coordinate care among patients, support networks, and healthcare providers to ensure positive outcomes

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
