Job Summary

A company is looking for an RN Care Navigator, 100% remote for New York residents.

Key Responsibilities

• Manage case management for kidney health, including patient data entry and care plan maintenance
• Educate members on kidney health and coordinate care with healthcare providers
• Advocate for patients and resolve barriers to care while utilizing community resources

Required Qualifications

• Active, unrestricted RN license
• Bachelor's degree
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
