New York RN Care Navigator

Job Summary

To support high-quality kidney health management, the remote New York RN Care Navigator will manage a caseload of patients, coordinate care delivery, and educate members on renal health while ensuring compliance with healthcare regulations.

Key responsibilities

• Handle inbound and outbound calls, providing exceptional service to members
• Educate patients on kidney health, dialysis options, and engage them in HealthMap's Kidney Health Program
• Coordinate care among patients, support networks, and healthcare providers to address individual needs and barriers

Required qualifications

• Active, unrestricted RN license
• Bachelor's degree or equivalent experience (6+ years of RN experience including 3+ years in case management may be considered in lieu of degree)
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
