New York RN Care Navigator

Job Summary

To facilitate high-quality kidney health management, the full-time remote New York RN Care Navigator will manage a caseload of patients, conduct health screenings, create care plans, and collaborate with healthcare teams to ensure effective care delivery.

Key responsibilities

• Handle in and outbound calls to provide exceptional service and educate members about kidney health and treatment options
• Engage members in HealthMap's Kidney Health Program and follow up based on policy guidelines
• Coordinate care by acting as a liaison between patients, their support networks, and healthcare providers

Required qualifications

• Active, unrestricted RN license in New York required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of a degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
