New York RN Care Navigator

Job Summary

To support kidney health management, the remote New York RN Care Navigator will manage a caseload of patients, facilitating care coordination, conducting health screenings, and developing member-centric care plans while ensuring high-quality outcomes.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service and education regarding kidney health and related conditions
• Engage and follow up with members in HealthMap's Kidney Health Program, addressing their needs and barriers to care
• Collaborate with interdisciplinary teams to coordinate care and advocate for patients' individualized goals

Required qualifications

• Active, unrestricted RN license in the state of New York
• Bachelor's degree required; or 6+ years of RN experience including 3+ years in case management may be considered in lieu of a degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
