New York RN Care Navigator

Job Summary

To support kidney health management, the remote New York RN Care Navigator will manage a caseload of patients, coordinating care and facilitating high-quality outcomes through education, advocacy, and collaboration with healthcare teams.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service and support to members
• Educate patients on kidney health, dialysis options, and related co-morbid conditions while engaging them in the Kidney Health Program
• Collaborate with interdisciplinary teams to coordinate care and address patient needs, ensuring effective communication and resolution of barriers

Required qualifications

• Active, unrestricted RN license in New York required
• Bachelor's degree required; 6+ years of RN experience, including 3+ years in case management may be considered in lieu of a degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
