Job Summary

A company is looking for an RN Case Manager.

Key Responsibilities

Conduct initial intake calls and ongoing care coordination for complex patients
Develop and implement care plans in collaboration with the patient's care team
Support patients during care transitions and ensure alignment with their needs

Required Qualifications

Current RN License in good standing in the state of practice
Bachelor's degree in nursing or associates in nursing with a related bachelor's degree
Minimum of 4 years clinical experience with at least 2 years in case management
Certified Case Manager (CCM) certification or commitment to obtain
Geriatric care experience is highly desired
