Job Summary

A company is looking for an RN Case Manager.

Key Responsibilities

Coordinate all aspects of a patient's care and serve as a patient advocate
Conduct initial intake calls and ongoing care coordination for complex patients
Participate in interdisciplinary care team meetings and quality improvement processes

Required Qualifications

Current RN License in good standing or unencumbered multistate license
Bachelor's degree in nursing or associates in nursing with another bachelor's degree
Minimum of 4 years clinical experience, with at least 2 years in case management
Certified Case Manager (CCM) certification or commitment to obtain
Geriatric care experience is highly desired
