New York RN Case Manager

Job Summary

A company is looking for an RN Case Manager Remote.

Key Responsibilities

• Coordinate care plans with physicians and multidisciplinary teams for high-risk members
• Assist members and families in obtaining referrals to specialists and provide tailored counseling
• Educate members about community and health plan benefits and services

Required Qualifications

• Bachelor of Science in Nursing (BSN) or 5 years of case management experience in lieu of BSN
• Unrestricted current RN licensure in New York or ability to obtain within 90 days of offer
• 2+ years of experience in health plan case management
• Experience in a remote and telephonic role
• Proficient in Microsoft Office and Adobe products
