New York RN Case Manager

Job Summary

To support a growing population health team, the full-time RN Case Manager will coordinate comprehensive patient care, engage in ongoing assessments, and collaborate with multidisciplinary teams while working remotely.

Key responsibilities

• Conduct initial intake calls and ongoing patient outreach to monitor care plans and address new concerns
• Provide care coordination for a panel of complex patients, ensuring patient-centered and aligned care
• Participate in interdisciplinary team meetings and quality improvement processes to enhance patient outcomes

Required qualifications

• Current RN License in good standing in New York or an unencumbered multistate license
• Bachelor's degree in nursing or an associate's degree in nursing with another bachelor's degree
• Minimum of 4 years of clinical experience, including at least 2 years in case management within home health or community health
• Certified Case Manager (CCM) certification or commitment to obtain when eligible
• Experience in geriatric care and knowledge of Medicare and Medicaid regulations preferred
