Job Summary

To support a leading healthcare company, the remote Bilingual RN Case Manager (English/Spanish) will conduct comprehensive telephonic assessments, develop individualized care plans, and coordinate services for members enrolled in Managed Long-Term Services and Supports (MLTSS) during a 6-month contract.

Key Responsibilities
  • Conduct comprehensive telephonic assessments for members to determine appropriate services and referrals
  • Develop individualized, cost-effective care plans that support members' health goals and coordinate care with various stakeholders
  • Maintain accurate documentation and follow case management processes in compliance with regulatory standards
Required Qualifications
  • Active, unrestricted Registered Nurse (RN) license in the State of New York
  • Associate degree in Nursing or higher
  • Minimum of 3 years of clinical experience in a relevant healthcare setting
  • Fluent in English and Spanish (speaking, reading, and writing)
  • Ability to work independently in a remote environment
