New York RN Case Manager

Job Summary

Utilizing a collaborative process of assessment and care coordination, the remote RN Case Manager in New York will manage comprehensive telephonic assessments, develop individualized care plans, and advocate for members to ensure access to appropriate healthcare services during a 6-month contract with potential for long-term employment.

Key Responsibilities

• Conduct comprehensive telephonic assessments for members enrolled in Managed Long-Term Services and Supports (MLTSS)
• Develop individualized, cost-effective care plans that align with members' health goals and coordinate care with various healthcare providers
• Maintain accurate documentation within the electronic health record and ensure compliance with regulatory and accreditation standards

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the State of New York
• Associate degree in Nursing or higher
• Minimum of 3 years of clinical experience in a relevant healthcare setting
• Strong assessment and care coordination skills
• Ability to work independently in a remote environment
