New York RN Case Manager

Job Summary

Utilizing a collaborative process for care coordination, the remote RN Case Manager will conduct comprehensive telephonic assessments, develop individualized care plans, and advocate for members' healthcare needs in a full-time contract role.

Key Responsibilities

• Conduct comprehensive telephonic assessments for members enrolled in Managed Long-Term Services and Supports (MLTSS)
• Develop individualized, cost-effective care plans that support members' health goals and coordinate care with various healthcare providers
• Maintain accurate documentation within the electronic health record and mentor new team members after achieving proficiency

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the State of New York
• Associate degree in Nursing or higher
• Minimum of 3 years of clinical experience in a hospital, home health, ambulatory care, or similar clinical setting
• Strong assessment, documentation, and care coordination skills
• Ability to work independently in a remote environment
