Job Summary
A company is looking for a Senior Care Manager (RN).
Key Responsibilities
- Develops and assesses ongoing care plans and collaborates with providers to address member's needs
- Coordinates care between members, families, and care providers to ensure appropriate services
- Monitors member status and revises care plans as necessary to meet goals and needs
Required Qualifications
- Active New York State RN License
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 4 - 6 years of related experience
- Knowledge of state and federal regulations related to healthcare
- Experience in care management or similar healthcare roles
