Key Responsibilities

Coordinates care between members, families, and providers to ensure person-centered services
Monitors member status and revises care plans as necessary to meet goals and needs

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
4 - 6 years of related experience
Must hold an RN license for New York
Experience with UAS assessments, intake, home health, hospice, and insurance preferred
