Job Summary

A company is looking for a Care Manager - DSNP - 100% Remote.

Key Responsibilities

• Plan and manage behavioral and/or physical care for members, ensuring coordination with healthcare teams
• Conduct assessments, develop care plans, and generate referrals to appropriate services
• Document all encounters and updates in accordance with established procedures

Required Qualifications

• NYS RN or LCSW/LMSW (any state) for Medical Care Management
• 3 years of work experience in a mental/behavioral health or addictions setting for Behavioral Health Care Management
• 1 year of pediatric clinical field experience for PEDS positions
• Credentialed Alcohol and Substance Abuse Counselor for CASAC positions
• Experience managing member information in a shared network environment
