Job Summary

A company is looking for a Care Manager, Bilingual Spanish - 100% Remote.

Key Responsibilities

• Advocates for and educates members on health services and self-management techniques
• Conducts assessments and develops care plans in collaboration with healthcare providers
• Coordinates care among multidisciplinary teams and documents all member interactions

Required Qualifications

• NYS RN or LCSW/LMSW (any state) for Medical Care Management
• 3 years of experience in a mental/behavioral health or addictions setting for Behavioral Health Care Management
• Experience with pediatric clinical field and child-serving systems for PEDS positions
• Fluency in Spanish is preferred
• Experience managing member information in a shared network environment
