• Assist with device setup and app connectivity while engaging with patients to ensure adherence to care plans
• Monitor patient data, schedule check-ins, and educate patients through shared resources

Qualifications

• Active New York State nursing license
• Knowledge of chronic conditions and related monitoring devices
• Proficiency with technology, computer platforms, and smartphones
• Experience in telehealth, remote patient monitoring, or customer service is preferred
• Independently motivated and comfortable with high-volume patient outreach
