Job Summary

To support the coordination of care for members in foster care, the full-time New York State Licensed Managed Care Liaison will manage communications with local departments, oversee care plans, and ensure compliance with healthcare requirements while working remotely.

Key responsibilities
  • Coordinate with the Office of Children and Family Services and Local Departments of Social Services to manage enrollment and care for foster care members
  • Assist in care coordination for members with chronic conditions or unstable health status, ensuring timely assessments and continuity of care
  • Participate in project management related to foster care operations and address issues affecting managed care members
Required qualifications
  • Bachelor's degree, preferably in Nursing or LMSW/LCSW
  • Current unrestricted New York State RN license and Licensed SW
  • CCM certification required, or completion within 180 days of hire
  • 4-6+ years of relevant work experience, preferably with a healthcare provider
  • Experience with the child welfare system and healthcare requirements for foster care populations
