Job Summary

Working full-time during days and evenings, the New York State Licensed Triage Nurse will manage telephone triage for clients and caregivers, facilitating healthcare access and ensuring client satisfaction while collaborating with the healthcare team.

Key responsibilities

• Triages calls from clients and caregivers, assessing conditions and formulating care plans based on established protocols
• Collaborates with team members to develop and implement effective plans of care
• Acts as an advocate for clients and families, providing non-judgmental advice and resolving concerns through effective communication

Required qualifications

• Graduate from an accredited school of nursing
• Current New York State Registered Nurse License
• Experience in emergency department or ambulatory care is preferred
• Knowledge of medical terminology
• 1 year of home health care experience is preferred
