Job Summary

Supporting administrative care management activities, the full-time Care Management Support Coordinator NICU will perform outreach, answer inbound calls, and schedule services while working remotely.

Key responsibilities
  • Conduct outreach to members to assist with care plan next steps and connect them to community or health plan resources
  • Serve as the front-line support for member and provider inquiries, explaining care plan procedures and protocols
  • Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required qualifications
  • High School diploma or GED
  • 1 - 2 years of related experience in a non-clinical role supporting clinical teams, preferably in NICU
  • Knowledge of health plan activities and resources
  • Familiarity with Social Determinants of Health (SDOH) needs
  • Ability to comply with policies and standards related to care management
