NJ Licensed Care Manager - Transplant

Job Summary

A company is looking for a RN II, Care Manager- Ortho/Transplant.

Key Responsibilities:

• Assess patient's clinical needs to ensure appropriate level of care and length of stay
• Coordinate and implement care plans while monitoring services rendered outside the network
• Advocate for members and families, ensuring effective resource utilization and quality care delivery

Required Qualifications, Training, and Education:

• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Minimum of two (2) years clinical experience required
• Minimum of two (2) years healthcare payer experience required
• Active Unrestricted NJ RN License or active Compact License required
• CCM certification preferred
