NJ Licensed Transplant Case Manager

Job Summary

Acting as a primary advocate and care coordinator, the full-time NJ Licensed Transplant Case Manager will navigate complex medical, social, and financial barriers for patients throughout their transplant journey, ensuring appropriate care access and support while working remotely.

Key responsibilities

• Assesses patients' clinical needs to ensure appropriate levels of care and length of stay
• Coordinates and implements care plans while monitoring services rendered and negotiating fees as necessary
• Documents patient care activities and collaborates with multidisciplinary teams to ensure continuity of care

Required qualifications

• Active Unrestricted NJ RN License or active Compact License required
• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Minimum of two (2) years clinical experience required
• Minimum of two (2) years' experience with health care payer experience required
• CCM certification preferred
