Non-Clinical Patient Advocate

Job Summary

A company is looking for an RN Patient Advocate, Remote 1099.

Key Responsibilities

• Serve as the primary contact for Medicare patients, building trusting relationships and supporting their understanding of care plans
• Coordinate care and connect patients with community resources to address social determinants of health
• Document patient interactions and maintain communication with supervising clinicians to ensure quality care

Required Qualifications

• 3+ years of experience in patient advocacy, care coordination, or healthcare navigation
• Strong understanding of the U.S. healthcare system, particularly Medicare
• Experience with older adults or medically complex populations
• Ability to work independently in a remote environment
• Demonstrated competencies in service coordination and patient advocacy
