Job Summary
Providing afterhours support, the part-time North Carolina Care Coordinator will conduct biopsychosocial assessments, make referrals to network providers, and document client interactions while working remotely on weekends from Friday to Sunday.
Key responsibilities
- Conduct biopsychosocial assessments by phone to identify client needs
- Make referrals to network providers based on client preferences and eligibility
- Document all activities in compliance with grant program guidelines and internal policies
Required qualifications
- High School Diploma/GED required; Associate degree or higher preferred
- Minimum of 2 years of experience working with at-risk populations, with a preference for experience with veterans
- Knowledge of resources available to veterans preferred
- Proficiency in web-based platforms and MS Office Suite
- Must reside in North Carolina or within 40 miles of the NC border
