Job Summary
A company is looking for an In Reach Coach to support individuals with serious mental health needs.
Key Responsibilities
- Engage members to provide education about permanent supportive housing and community resources
- Collaborate with staff to facilitate transition planning for members seeking permanent supportive housing
- Utilize person-centered planning methods to address individual needs and provide education on rights and responsibilities
Required Qualifications, Training, and Education
- High School Diploma or GED required
- Must have at least one year of lived experience with a behavioral health condition and/or substance use
- Must be fully vaccinated against Covid-19 and other specified immunizations
- Must be a North Carolina Certified Peer Support Specialist or obtain certification within six months of employment
- Must have a valid driver's license and reside within 75 miles of Wayne County, North Carolina
