Job Summary

A company is looking for an In Reach Coach to support individuals with serious mental health needs.

Key Responsibilities

• Engage members to provide education about permanent supportive housing and community resources
• Collaborate with staff to facilitate transition planning for members seeking permanent supportive housing
• Utilize person-centered planning methods to address individual needs and provide education on rights and responsibilities

Required Qualifications, Training, and Education

• High School Diploma or GED required
• Must have at least one year of lived experience with a behavioral health condition and/or substance use
• Must be fully vaccinated against Covid-19 and other specified immunizations
• Must be a North Carolina Certified Peer Support Specialist or obtain certification within six months of employment
• Must have a valid driver's license and reside within 75 miles of Wayne County, North Carolina
