Job Summary
To enhance community health initiatives, the full-time remote North Carolina Health Outreach Specialist will conduct targeted member outreach, educate on available health benefits, and collaborate with community organizations to improve access to care.
Key responsibilities
- Conduct proactive outreach using various communication methods to engage members in health promotion campaigns
- Educate members on preventive care and assist in overcoming barriers to accessing healthcare services
- Plan and implement community health promotion initiatives in collaboration with stakeholders to address priority health needs
Required qualifications
- Bachelor's degree in public health, health promotion, or related field with three years of relevant experience, or an Associate's degree with five years of experience
- Experience working with individuals with behavioral or physical health needs, emphasizing cultural responsiveness and health literacy
- Knowledge of Medicaid benefits and community health promotion strategies
- Proficiency in Microsoft Office and experience with electronic medical records
- Valid driver's license
