Job Summary
A company is looking for an IDD In Reach & Transition Coordinator (Remote NC).
Key Responsibilities
- Identify consumers/enrollees for potential local community placement from State and Local Hospitals and facilities
- Assist in the transition process by matching consumers/enrollees with available housing and coordinating support services
- Document in-reach and transition progress accurately for each individual, ensuring compliance with local, state, and federal requirements
Required Qualifications and Education
- Bachelor's degree in public service administration, psychology, sociology, social work, or a related field with four years of relevant experience, or a Master's degree with two years of supervised clinical experience
- Experience in case management, assessment and referral, supportive counseling, intervention, psycho-social therapy, and treatment planning
- Experience working with children, adolescents, and adults with mental health and intellectual/developmental disabilities
- Ability to establish respectful relationships with clients and their families
- Residency in North Carolina or within 40 miles of the North Carolina border is required
