North Carolina Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Behavioral Health Case Manager - Medicaid.

Key Responsibilities

• Coordinate care, resources, and services for D-SNP member populations to achieve optimal clinical outcomes
• Develop and manage member-centric care plans, including comprehensive assessments and ongoing monitoring
• Educate members and facilitate access to community resources while documenting all aspects of care

Required Qualifications

• RN, RD, or LCSW with a minimum of 3 years of clinical and/or case management experience
• Valid and applicable clinical license (NC or compact multi-state licensure) is required
• Additional specialty certification may be required within 2 years of employment
• Experience in D-SNP is a plus
