Job Summary
To support a multidisciplinary care management team, the full-time remote North Carolina Licensed Care Consultant will provide secondary case management for members with physical health needs, offer on-demand recommendations, complete medication reviews, and collaborate with healthcare providers to optimize health outcomes.
Key responsibilities
- Provide care team support and expertise regarding members' physical and behavioral health
- Utilize person-centered planning and assessments to develop and implement care plans
- Manage medication reviews and reconcile medications while consulting with the Pharmacy Team as needed
Required qualifications
- Active, valid RN license in North Carolina or active, valid OT/PT license in North Carolina
- Graduation from an accredited nursing school with three years of experience in care management, or a master's degree in Occupational or Physical Therapy with two years of experience
- Demonstrated knowledge of mental health assessment and treatment
- Proficiency in Microsoft Office products
- Ability to independently manage caseloads and prioritize tasks effectively
