North Carolina Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator (Remote NC).

Key Responsibilities

• Conduct telephonic outreach and engagement for members eligible for Tailored Care Management and provide care coordination
• Administer screenings and assessments, develop care plans, and manage transitions of care to ensure members receive appropriate services
• Document activities in designated software platforms according to policy and procedure

Required Qualifications, Training, and Education

• Bachelor's Degree in Human Services or a related field with at least two years of relevant experience, or a Bachelor's Degree in another field with four years of relevant experience
• Qualified Professional status according to 10A NCAC 27G .0104 is required
• Experience in call center operations, care coordination, or social determinants of health is preferred
• Knowledge of behavioral health services and whole-person health concepts is essential
• Proficiency in Microsoft Office products is required
