North Carolina Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator (Remote NC).

Key Responsibilities

• Conduct telephonic outreach and engagement for members eligible for Tailored Care Management
• Provide telephonic assessments and develop person-centered care plans for members
• Manage transitions of care and link members to appropriate services to meet their care goals

Required Qualifications, Training, and Education

• Bachelor's Degree in Human Services or a related field
• At least two (2) years of experience providing similar services (or four (4) years in a non-Human Services field)
• Qualified Professional status according to 10A NCAC 27G .0104
• Experience in call center operations and care coordination preferred
• Knowledge of Behavioral Health/I/DD Tailored Plan eligibility and services
