North Carolina Licensed Care Management Manager

Job Summary

A company is looking for a Manager of Care Management Support.

Key Responsibilities

• Oversee care coordination teams, ensuring compliance with policies and efficiency in operations
• Develop relationships with internal and external stakeholders to enhance collaboration and resolve conflicts
• Implement quality improvement strategies and oversee program operations and documentation maintenance

Required Qualifications

• Master's degree in Counseling, Social Work, or related field with four years of relevant experience, or a Bachelor's degree in Nursing with five years of experience
• Two years of supervisory experience
• Licensure in North Carolina as LCSW, LCMHC, or LMFT, or as a Registered Nurse
• Knowledge of care management for complex individuals with behavioral and/or physical health conditions
• Preferred certifications include NACCM, NADD-Specialist, and/or CBIS
