North Carolina Licensed Care Manager

Job Summary

A company is looking for a Care Manager to work with clinical teams and community resources to provide care management services.

Key Responsibilities

• Develop and monitor Tailored Care Management care plans and Individual Support Plans (ISPs)
• Engage members and families to ensure service provision and maintain positive relationships with stakeholders
• Conduct comprehensive health assessments and coordinate care team meetings to address member health needs

Required Qualifications

• Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred
• Must meet all agency requirements for pre-employment and state requirements for North Carolina
• A license or certification in a human service profession or a relevant degree with experience in the IDD population
• Experience in LTSS or HCBS coordination and care management is preferred
• Ability to work independently with a high degree of quality
