North Carolina Licensed Care Manager

Job Summary

A company is looking for a Care Manager.

Key Responsibilities

• Develops and maintains positive relationships with members, families, and care teams to enhance health and quality of life outcomes
• Conducts comprehensive health assessments and develops, implements, and evaluates care plans for members
• Coordinates care team meetings and ensures effective communication among all stakeholders involved in the care process

Required Qualifications

• Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred
• Must meet all agency and state requirements for pre-employment
• A license or certification in a human service profession is required
• A Master's degree in a human service field with one year of relevant experience, or a Bachelor's degree in a human service field with two years of relevant experience is preferred
• Experience with individuals with intellectual and developmental disabilities (IDD) is necessary
